Summary – RN

Under the general supervision of the RN Manager, she/he provides intermittent skilled nursing services; communicates the patient's progress with other disciplines and directs, supervises and instructs non-professional home health aide staff in the provision of personal care to the patient. .

Responsibilities – RN:

Under the physician's order, admits patients eligible for home care services.

* Develops patient care plan that specifically addresses identified patient problems; nursing problems and goals. Updates care plans on an ongoing basis; revises and resolves patient problems and goals as changes occur and/or recertification..
* Provides intermittent Skilled Nursing services including assessment, evaluation, procedures, teaching and training activities as outlined in the patient Plan of Treatment.
* Submits completed skilled nursing notes; communication notes and home care aide supervisory notes per policy.
* Submits recertification paperwork by the due date provided by the RN Manager.
* Effectively communicates with all members of the healthcare team.
* Acts as the patient's advocate and as such are a liaison to assist in communicating the patient's needs to the multidisciplinary team.
* Supervises the hospice aide every 14 days. Provides direction and instruction as it relates to provision of personal care and related support services.
* Participates in PI program through submission of data collection as it relates to direct patient care problems and serving on PI teams.
* Participates in discharge planning process.
* Additional Duties as assigned

* One year experience as a home care nurse, including OASIS experience
* Ability to work in a field setting and exhibited ability to make sound nursing judgments

On the Bridges inpatient psychiatry unit, the registered nurse provides care to young, middle and older adults with psychiatric and medical diagnoses on 25-bed locked inpatient unit. The RN provides care to medically stable psychiatric patients who often require ongoing monitoring and physical assessment of chronic conditions (such as Diabetes, hypertension, COPD, et al) concurrently with acute psychiatric care. Participates as an interdisciplinary team member in treatment and aftercare planning of the psychiatrically ill patient. Provides patient and family education for illness and medication management.

Approved LPS designated staff member, will perform psychiatric evaluations, provide support and assistance in problem resolution, assess mental status and potential for 72 hour hold for grave disability, danger to self or danger to others and evaluate the level of care required for treatment for each individual case and coordinate the provision of needed services. Will function as an LPS Designated professional on a rotating basis with peers, and participate in the L.A. County-based training as well as the credentialing process in the Medical Staffing department as coordinated and agreed upon with Nurse Manager.
Minimum requirements:
A.Must have a valid California Registered Nurse license and current AHA CPR certification.
B.At least 6 months of medical-surgical nursing experience with recent direct patient care in an acute care facility. Must have some experience in psychiatry, preferably inpatient setting.
C.Must have current or prior MAB, PART, or CPI certification, with an agreement to retrain in CPI within 2 months of hire.
D.Psychiatric Evaluation Team Member: Minimum of three years of acute inpatient psychiatric experience. LPS designated by the authority of Los Angeles County of Mental Health Services. Must be approved by Medical Staff Allied Health Section, meet requirements and maintain good standing of the hospital?s medical staff. Must have knowledge of the following: fundamentals of basic nursing care, psychiatric nursing practice, general psychiatric emergency intervention and laws and regulations related to patient?s rights, voluntary or involuntary treatment and criteria for commitment. Familiar with community resources and agencies providing support services for the mentally ill or disabled.
